General principles in Radio-isotope scanning, istopes, conclusions.?
General Principle: Radio-Isotopes are taken up by cells in the body, just as their non-radioactive counterparts are, however with the advantage of being able to be visualized with an intense signal when bombarded with x-rays. Now, depending on the type of study you are doing you will use a different injectable substance and you would expect to see different results depending on the pathology. I will use the example of a bone scan and a thyroid scan to make this point. In a bone you use technetium-99 and this will be taken up and concentrated in cells that are more active than others. For example, areas of normal growth (growth plates in a child or sites of a fracture), areas of infection (osteomyelitis), or neoplasia will light up. This test along with your clinical history will fill your Dr in on what your problem is. In a thyroid scan, typically, I 131 is used. Here, the busier cells are at making thyroid hormone (T3/T4), the more intensely they will light up. In the case of Graves disease, the thyroid will light up in a diffuse fashion where as in some forms of cancer, the thyroid will show nodular areas that do not light up at all because these cells lost the ability to take up I 131. In conclusion, these test are used to direct the Dr ordering them into the next step which is either prognosis, diagnosis, or disease management. Like any test there are risks and the radiologist will explain this to you and your Dr will do things as to minimise your risk profile.
